Brow Lift Gone Wrong: Common Problems and Fixes

A brow lift gone wrong most often means brows that sit too high or look surprised, uneven brows, a raised hairline, visible scars, hair loss at incisions or long-lasting numbness. Many problems settle as swelling fades over a few months. Those that remain can often be improved with Botox, a small revision or targeted scar and hair treatment.

This guide belongs to our brow lift overview. It explains what can go wrong, what’s normal during healing and what can be done.

Common brow lift complications

Each technique has its own typical problems, from numbness after direct lifts to hair loss after endoscopic lifts and revisions after hairline lifts.

The American Society of Plastic Surgeons (ASPS, US) lists these brow lift risks: bleeding, a raised hairline, eye irritation or dryness, asymmetry, facial nerve injury with weakness, fluid build-up, infection, hair loss around incisions, numbness or intense itching, persistent pain, poor healing, skin loss, unfavourable scarring and the possible need for revision.

A 2018 review of 76 studies in Plastic and Reconstructive Surgery Global Open found the highest rate of each problem varied by technique:

Technique Most notable problem Rate
Hairline Revision surgery 7.4%
Direct Numbness 5.5%
Endoscopic Hair loss at incisions 2.8%
Temporal (lateral) Asymmetry 1.5%

In a 2015 series of 37 temporal brow lifts, three patients had temporary weakness of the forehead nerve that resolved within three to six months, and one had a small patch of hair loss. Our brow lift types comparison explains each technique.

Surprised look

An over-lifted or over-arched inner brow causes the “surprised” look; it’s often worst early on and may soften as swelling settles.

Brows commonly sit higher in the first weeks because of swelling, then drop slightly. If the inner brow was lifted too much, the face can look startled or worried. A brow lift fail can also come from Botox: too much toxin low in the forehead can drop the brows, while toxin only between the brows can leave the outer brows peaked.

In my practice, I keep the inner brow lift conservative and wait at least three months before judging the final height. Many patients who worry in week two are happy by month three.

Fixes

Fixes range from waiting and Botox balancing to scar treatment, hair restoration and revision surgery, depending on the problem.

  • Brows too high or uneven: wait for swelling to settle. Small differences can be balanced with Botox. Persistent large differences may need revision surgery.
  • Brows dropped again: fixation can loosen over time. An ASPS article notes a browpexy done through the eyelid may start to fall after a few years. A further lift or a different technique may help.
  • Raised hairline: a hairline-lowering approach or hair restoration can be considered once healing is complete.
  • Hair loss at incisions: some shedding often regrows within months; persistent patches can be treated with scar revision or hair transplant.
  • Visible scars: scar treatments, revision or, for hairline scars, hairstyle changes.
  • Numbness and itching: usually improve over months as nerves recover; persistent symptoms need review.
  • Forehead nerve weakness: usually temporary; recovery can take months.

See a surgeon urgently for sudden swelling, fever, discharge or chest pain. If you’re planning surgery, the brow lift recovery guide shows what’s normal, and our before and after guide explains what a natural result looks like.

Frequently Asked Questions

FAQ

Short answers on brow lift fail problems, timing of revisions, Botox mistakes, numbness, hair loss and choosing a surgeon.

How long should I wait before judging a brow lift?

Most surgeons advise waiting about three months, and some longer. ASPS says the final result appears over several months as swelling settles. Brows often sit too high at first and drop slightly during healing.

Can a brow lift that went wrong be fixed?

Often, yes. Small asymmetry can be balanced with Botox. Brows that remain too high, too low or uneven may need revision surgery once healing is complete. Scars and hair loss have their own treatments.

Why do my brows look surprised after Botox?

Usually because the outer forehead muscle was left active while the centre was relaxed, peaking the outer brow. A small top-up dose can soften it. The effect also fades as the toxin wears off over about three to four months.

Is numbness after a brow lift permanent?

Usually not. Numbness and itching on the scalp often improve over months as nerves recover. A 2018 review found numbness most often after direct brow lifts, at 5.5%. Ask your surgeon to review symptoms that last beyond a year.

Will hair grow back around my incisions?

Some shedding near incisions is common and often regrows within a few months. Endoscopic lifts had a 2.8% rate of hair loss in a 2018 review. Persistent patches can be treated with scar revision or hair transplant.

How can I lower my risk of a bad result?

Choose a board-certified surgeon experienced in brow lifts, agree on the planned brow height beforehand, don’t smoke, and follow aftercare closely. Ask to see results from patients with a similar brow and hairline to yours.
