Breast lift vs augmentation comes down to whether the problem is position or volume. A lift raises the nipple and tightens loose skin but doesn’t add size. Augmentation adds volume with an implant or fat but doesn’t raise a low nipple. If your breasts have both dropped and emptied, you may need both.
This guide is part of our breast lift overview.
Quick answer
Choose a lift for drooping, augmentation for small or empty breasts with a good nipple position, and both for drooping plus volume loss.
- Lift: nipple at or below the breast fold, stretched skin, enlarged areola, breasts pointing down.
- Augmentation: nipple above the fold, breasts small or deflated, upper breast looks empty.
- Both: nipple has dropped and you also want more fullness.
The American Society of Plastic Surgeons (ASPS, US) says augmentation alone doesn’t correct severely drooping breasts, while a lift doesn’t significantly change size or round out the upper breast.
Comparison table
Breast lift vs implant: one reshapes and raises, the other fills; they differ in scars, cost and long-term upkeep.
| Factor | Breast lift | Augmentation (implants) |
|---|---|---|
| Main change | Raises nipple, removes extra skin, reshapes | Adds volume and upper fullness |
| Size | Little change | Larger |
| Scars | Around areola, often down to and along the fold | Short incision in fold, areola edge or armpit |
| US surgeon fee (ASPS 2024) | $6,500 to $11,000 | $4,575 to $8,000 |
| Long-term upkeep | Breasts keep aging; stable weight helps | Implants aren’t lifetime devices; silicone screening scans |
| Main specific risks | Nipple sensation change, scarring, rare nipple loss | Capsular contracture, rupture, rippling, rare BIA-ALCL |

Pinch test and ptosis grade
Surgeons judge sagging by nipple position against the breast fold and measure skin stretch; these numbers guide lift versus implant.
StatPearls describes the Regnault grades of sagging (ptosis):
- Grade 1 (mild): nipple level with the breast fold
- Grade 2 (moderate): nipple below the fold but not at the lowest point of the breast
- Grade 3 (severe): nipple below the fold and at the lowest point
- Pseudoptosis: nipple at or above the fold, but breast tissue hangs below it
StatPearls also notes that augmentation alone suits people with little or no sagging, for example a nipple-to-fold distance under 10 cm or skin stretch under 4 cm. A surgeon’s pinch test checks how much tissue covers an implant. At home, you can stand side-on to a mirror without a bra and see where your nipple sits compared with the crease under your breast, but only an examination can confirm the grade.
When you need both
If the nipple has dropped and the upper breast is empty, a lift with an implant or fat often gives the result people picture.
StatPearls reports a reoperation rate of 10.7% for one-stage lift with implants, close to 10.2% for a lift alone, in a meta-analysis of 4,856 cases. Staging is preferred for severe sagging or large implants. Read our guides to a breast lift with augmentation and a breast lift with fat transfer.
A situation I see often is someone asking for implants to fix sagging. An implant alone in a breast that has dropped can sit high while the tissue hangs over it, so I explain why a lift needs to be part of the plan.
Frequently Asked Questions
FAQ
Answers on fixing mild sag with implants, choosing a lift without implants, cost differences and recovery.
Can implants fix mild sagging?
Sometimes. With mild sagging or pseudoptosis, an implant can fill loose skin and improve the shape. ASPS says augmentation doesn’t correct severe drooping, so if your nipple sits well below the fold, a lift is usually needed.
Can I have a lift without implants?
Yes. A lift alone suits people happy with their size who want a higher, firmer shape. It can make breasts look slightly smaller. If you’d like a little more upper fullness without an implant, fat transfer is an option.
Which costs more, a lift or augmentation?
In the US, ASPS 2024 surgeon-fee ranges are $6,500 to $11,000 for a lift and $4,575 to $8,000 for implants. Facility, anesthesia and implant costs are extra, and combining both raises the total.
Is recovery different?
Both usually need a support bra for several weeks. For a lift, StatPearls suggests a support bra for four to six weeks and intense exercise from about four to six weeks. Implants add some chest tightness early on, especially under the muscle.
If you’d like your sagging grade and tissue measured, you can book a consultation with Metropole Clinic by ABmedi. For techniques and scars, see our breast lift guide.
