A breast reduction with lift removes tissue to make breasts smaller and repositions what remains higher on the chest. In practice, almost every surgical reduction includes a lift, because the nipple is moved up and skin is tightened. You need extra attention to the lift when skin is very loose, such as after weight loss or pregnancy.
This guide is part of our breast reduction overview. For a lift on its own, see our breast lift guide.
Reduction vs reduction + lift
Breast reduction with lift is the norm, not an add-on: removing tissue and raising the breast happen in the same operation.
The American Society of Plastic Surgeons (ASPS, US) defines breast reduction as removing skin and tissue, then reshaping and elevating the breast. ASPS’s breast lift pages suggest combining a lift with a reduction when someone wants smaller breasts as well as a higher position.
| Factor | Lift alone | Reduction (with lift) |
|---|---|---|
| Main goal | Higher position, firmer shape | Smaller, lighter and higher breasts; symptom relief |
| Tissue removed | Mainly skin | Breast tissue, fat and skin |
| Size change | Little | Clear reduction |
| US surgeon fee (ASPS 2024) | $6,500 to $11,000 | $7,000 to $12,500 (cosmetic) |
| Insurance | Usually not covered | Often covered with documented symptoms |
| Scars | Around areola, often vertical, sometimes along fold | Usually lollipop or anchor |
If your main issue is sagging and you’re happy with size, a lift alone may be enough. If heaviness causes pain, a reduction is the better fit.
Before and after
Typical breast reduction and lift before after photos show smaller breasts sitting higher, with the nipple centred and the areola often made smaller.
What a combined result usually shows:
- A higher nipple, centred on the breast
- A narrower areola, as ASPS describes removing skin from its edge
- Less fullness hanging below the fold
- Strap grooves and rashes improving
- Scars around the areola and down to or along the fold
ASPS says reduction results are long-lasting, though aging, weight, hormones and gravity still change the breasts. A 2022 systematic review in JPRAS Open of 24 studies looked at how long sagging correction lasts after lifts and reductions, showing this is an active research area. For what to look for in photos, see breast reduction before and after.
When you need both
Combined breast reduction and lifting suits people with heavy breasts and loose skin, especially after major weight loss or pregnancy.
Weight loss can make breasts smaller but leave loose skin, and a plastic surgeon writing for Nebraska Medicine notes that reduction may be combined with a lift in that situation. Signs that the lift part matters most:
- Nipple well below the breast fold
- Thin, stretched skin with stretch marks
- Breasts that look empty at the top but hang low
In my practice, I explain to patients that the size decision and the lift decision are separate questions, and we agree on both before surgery.
Recovery and scars
Recovery follows the reduction timeline: two to three weeks off work, no heavy lifting for up to six weeks and swelling for about three months.
The National Health Service (NHS, UK) says reduction usually means one to two nights in hospital, two to three weeks off work and avoiding strenuous exercise and heavy lifting for up to six weeks. Scars may be red for the first six weeks, then most fade. The NHS lists scars around the nipple, often with a vertical scar and sometimes an anchor-shaped scar. See our breast reduction recovery guide.
Frequently Asked Questions
FAQ
Answers on whether a reduction always lifts, insurance for a combined operation, scars and how long results last.
Does a breast reduction always include a lift?
Almost always. ASPS describes reduction as removing tissue and then reshaping and elevating the breast, and the nipple is usually moved higher. How much lifting is needed depends on how far the breast has dropped and how loose the skin is.
Will insurance pay for a reduction with a lift?
Insurers that cover breast reduction for documented symptoms usually treat the reshaping as part of the operation. A purely cosmetic lift without reduction is generally not covered. Check your policy and pre-approval terms.
Are the scars bigger than for a reduction alone?
Usually not, because both use the same lollipop or anchor patterns. The scar length depends on how much skin must be removed. Scars are permanent but usually fade over months.
How long do the results last?
Long-lasting, according to ASPS. Removed tissue doesn’t return, but weight gain, pregnancy, hormones and gravity can change size and position over time. A stable weight helps keep the result.
