Facelift Before and After: Natural Results You Can Expect

Facelifts before and after photos usually show a firmer jawline, smaller jowls and a smoother neck, while the person still looks like themselves. A natural result keeps your expressions, hides the scars around the ear and avoids a tight or swept-back look. The final shape takes six to nine months to settle.

This guide is part of our complete facelift overview. It explains what a realistic change looks like, how it tends to differ by decade and how to tell a natural result from an overdone one.

Signs of a good facelift

A good facelift gives a smoother jaw and neck, keeps normal expression and ear shape, and leaves scars that sit in the hairline and natural ear folds.

Start with the jawline. After a well-planned lift the line from chin to ear is cleaner, the jowl is smaller and the neck angle is sharper. The cheek should look supported, not flattened. You should still be able to smile, frown and raise your brows the way you did before, once the early swelling has gone.

Then look at the details friends won’t notice but surgeons do. The American Society of Plastic Surgeons (ASPS, US) says healed facelift scars should be well concealed in the hairline and the natural contours of the ear. The earlobe should hang freely, the sideburn should stay in place and the hairline behind the ear shouldn’t show a step.

What to check Natural result Warning sign
Jawline Smoother, jowls reduced Jowls unchanged or a hollow below the cheek
Cheeks and mouth Supported, mouth corners unchanged Cheeks pulled sideways, wider-looking mouth
Earlobe Hangs freely in its old position Pulled down and stuck to the cheek (“pixie ear”)
Hairline and sideburn Unchanged position Raised sideburn or a step in the hairline
Scars Thin, in the ear folds and hair Wide, raised or visible in front of the hairline

The warning signs in this table come from the list of poor-outcome features in a 2026 StatPearls review on NCBI Bookshelf (US).

What I tell patients before surgery is that the compliment to aim for after a facelift is “you look well”, not “who did your face”. That’s the result I plan for.

Facelifts before and after by age

Facelifts before and after photos look different by decade: at 40 the change is subtle, at 50 the jawline shifts most, and after 60 the neck often needs work too.

Age alone doesn’t decide the result. The amount of laxity, skin quality and bone support matter more. Still, some patterns are common.

In your 40s

Changes are usually early: a softening jawline and the first hint of jowls. A StatPearls review describes short-scar lifts as typically suited to people around 40 to 50 with limited aging and no severe neck laxity. The before and after difference is real but modest, which is often exactly what people at this stage want. Our mini facelift guide covers this option.

In your 50s and 60s

This is when facelifts are most common. In the ASPS 2024 statistics, people aged 55 to 69 accounted for 59% of US facelifts. Jowls and neck laxity are usually clearer, so the visible change is larger. Deeper folds beside the mouth tend to soften, though they rarely disappear.

Over 70

People 70 and older had 20% of US facelifts in 2024 (ASPS). Thinner skin and deeper neck changes are common at this stage. Health screening carries more weight, and the gain is often most obvious in the neck. For decade-by-decade detail, read facelift at 40, 50 or 60.

Natural facelifts before and after vs overdone results

Natural results come from lifting the deeper layer and closing skin without tension. Overdone results usually come from pulling skin sideways or too tight.

Skin isn’t strong enough to hold a lift by itself. When a surgeon relies on skin tension, the face can take on a pulled, windswept look, and the scars may stretch. A 2020 review of 72 revision facelifts in Plastic and Reconstructive Surgery Global Open found that a shifted hairline was the most frequent problem (48 of 72 cases), and pixie ear deformity appeared in 10. The authors linked widened scars to skin-only lifts closed under tension.

Timing also changes how a photo looks. Swelling hides detail early on, and the National Health Service (NHS, UK) says the final result can take six to nine months. Photos taken at two weeks and at nine months can look quite different.

How to read before-and-after photos

  • Same angle, light, expression and distance in both shots. A lifted chin flatters any jawline.
  • A side view and a three-quarter view, not just the front.
  • A stated time since surgery, ideally six months or more.
  • Visible ears and hairline, so you can judge scars and earlobe position.
  • Real, consented patients only. Results shown on websites should never be computer-generated.

Frequently Asked Questions

FAQ

Quick answers on when facelift results show, how long they last and what natural before and after photos should look like.

When will I see my final facelift result?

Most people look presentable within 10 to 14 days, according to ASPS. The face takes two to three months to feel normal again, and the NHS says the final result can take six to nine months to show as swelling fully settles.

What do natural facelifts before and after look like?

A natural result shows a cleaner jawline, smaller jowls and a smoother neck, with normal expressions. The earlobe and sideburn stay where they were, the mouth corners aren’t pulled sideways, and scars sit in the hairline and ear folds where they are hard to see.

How long do facelift results last?

In a study reported by ASPS, about 76% of 50 patients still looked younger than before surgery at an average of 5.5 years. The neck lost the most improvement over time. Your face keeps aging, so results soften gradually rather than ending suddenly.

Why do some facelifts look pulled or windswept?

A windswept look usually comes from too much sideways tension on the cheek or from relying on skin to hold the lift. Lifting the deeper SMAS layer and closing the skin without tension lowers the risk. Revision can often improve the look once tissues settle.

Can I trust before and after photos online?

Trust them only when the angle, lighting and expression match, the time since surgery is stated, and the ears and hairline are visible. Ask your surgeon to show you their own consented patient photos of people with a similar starting point to yours.
