A breast lift fat transfer combines a mastopexy with your own fat, taken by liposuction and injected to fill the flat upper breast. Auto-augmentation does a similar job by reshaping your own breast tissue upward. Both avoid implants but add only modest volume, so they suit people wanting lift and natural fullness, not a size jump.
This guide is part of our breast lift overview.
Auto-augmentation explained
Breast lift auto augmentation moves lower breast tissue up to fill the top of the breast, using what you already have instead of an implant.
During auto-augmentation, the surgeon keeps tissue from the lower breast that would otherwise be removed, folds it upward behind the nipple and secures it as an internal fill. A 2015 series in Archives of Plastic Surgery reported 107 such procedures in 53 patients, with an average nipple lift of 8 cm, uncomplicated healing in 94.3% and 5.7% unhappy with the lift level and needing more surgery.
A breast lift and fat transfer works differently. According to an American Society of Plastic Surgeons (ASPS, US) surgeon blog, fat is taken from the abdomen, thighs, flanks or bra line, processed and injected into the breast. That surgeon describes the gain as a subtle increase, useful for filling the upper breast after breastfeeding.
Breast lift fat transfer vs implant lift
A fat or tissue lift gives soft, natural fullness with no device; an implant lift gives more predictable volume but adds implant upkeep.
| Factor | Lift + fat transfer | Lift + auto-augmentation | Lift + implant |
|---|---|---|---|
| Volume added | Modest; part of fat is lost | None added; tissue redistributed | Chosen in advance |
| Extra procedure | Liposuction | No | No |
| Device upkeep | None | None | Implants aren’t lifetime devices |
| Specific risks | Fat necrosis, calcification | Nipple blood-supply problems, hematoma | Capsular contracture, rupture, recurrent sag |
| US surgeon fee (ASPS 2024) | Lift $6,500 to $11,000 plus fat grafting cost | Within lift fee | Lift plus implant fee ($4,575 to $8,000) |

Results
Breast lift with fat grafting results show a lifted shape with softer upper fullness; how much fat survives decides the final volume.
In a 2024 systematic review of fat grafting to the breast (35 studies, 3,757 women), an average of 58% of injected fat was retained, with a 27.8% complication rate, mostly fat necrosis. Satisfaction averaged 92% at one year. Those figures come from fat-only augmentation, not combined lifts.
Evidence for the combined operation is smaller. A 2024 case series in Aesthetic Surgery Journal Open Forum of 20 patients who had a lift with implants plus fat grafted in several layers reported no complications over about two years, but it was a small, low-level study.
In my experience, patients happiest with a fat lift are those who mainly want their upper breast filled and their shape restored, not a larger cup.
Candidates
Good candidates have mild to moderate sagging, want natural fullness rather than size and, for fat transfer, have enough spare fat.
- Sagging that needs a lift, with an empty-looking upper breast
- Happy with current size, or wanting only a small increase
- Enough fat on the abdomen, thighs or flanks (for fat transfer)
- Prefer to avoid an implant
- Healthy, non-smoking and at a stable weight, as ASPS advises for any lift
- Finished having children, since pregnancy can reverse a lift
The ASPS surgeon blog notes most patients keep nipple sensation and the ability to breastfeed, though sensation may be reduced for a while. For recovery, see the breast lift recovery timeline.
Frequently Asked Questions
FAQ
Answers on size gain, permanence, repeat sessions and how fat transfer compares with implants after a lift.
How much bigger will a fat transfer lift make my breasts?
Only slightly. Fat mainly fills the upper breast. In fat-only augmentation studies, on average just over half the injected fat survived. If you want a clear size increase, an implant lift is more predictable.
Is fat transfer with a lift permanent?
Fat that survives the first months generally stays and changes with your weight. The lift itself is long-lasting, but breasts continue to age, and pregnancy or large weight loss can reduce the effect.
What is the difference between auto-augmentation and fat transfer?
Auto-augmentation reshapes your existing lower breast tissue upward, so no fat is moved and no liposuction is needed. Fat transfer adds fat from elsewhere in your body. Some surgeons combine both.
Might I need more than one fat transfer?
Possibly. Because part of the fat is reabsorbed, some people choose a second grafting session months later. Ask your surgeon how much fat they plan to place and how they will judge whether more is needed.
