Breast Lift Before and After Without Implants (Mastopexy Results)

Mastopexy without implants before and after photos typically show a higher nipple, a smaller or reshaped areola, a firmer, rounder breast that sits higher on the chest, and scars that fade over months. Size stays about the same or slightly smaller. The upper breast gains only modest fullness, because a lift reshapes existing tissue rather than adding volume.

This guide is part of our breast lift overview. We don’t show real patient photos here; instead, this page explains what to look for in a surgeon’s own gallery.

Mastopexy results without implants

Natural breast lift results without implants raise and reshape; they don’t add size or fully fill a flat upper breast.

According to the American Society of Plastic Surgeons (ASPS, US), results are visible immediately, but swelling affects the early look and the final shape settles over a few months. A lift doesn’t significantly change size or round out the upper breast.

When you look at breast lift images or mastopexy before after galleries, use this checklist:

Check Why it matters
Same angles and lighting Posture and light can fake lift
Starting grade similar to yours A mild sag gives a different result from a severe one
Nipple centred on the breast Too high a nipple is hard to correct
Photos 6 months or more after surgery Early photos include swelling; shape keeps settling
Scars visible in the image Shows you the real trade-off

A 2015 series in Archives of Plastic Surgery of 107 lift procedures that reshaped the patient’s own tissue to add upper fullness (auto-augmentation) reported nipple elevation of 6 to 12 cm, uncomplicated healing in 94.3% of patients and 5.7% wanting further surgery.

Donut lift results

Donut breast lift before and after results are subtle: the nipple rises by up to about 2 cm, with a scar only around the areola.

StatPearls says the periareolar (donut) lift suits mild to moderate sagging and moves the nipple by no more than about 2 cm. It can also flatten the breast slightly. The scar sits at the areola edge, but StatPearls notes this technique has a high dissatisfaction rate, and a 2002 survey of surgeons found revision rates of 50% for periareolar lifts, compared with 29.9% for vertical and 21% for anchor lifts. If your sagging is more than mild, a bigger incision often gives a more lasting shape.

Internal bra breast lift before and after results

“Internal bra” lifts add mesh or tissue support inside the breast to slow sagging, but long-term evidence is limited.

Some surgeons place a synthetic or biological mesh, or use the patient’s own tissue, as an internal sling. A 2022 systematic review in Aesthetic Plastic Surgery found only ten relevant studies on mesh support after lifts and reductions, all at evidence level III. A separate 2022 systematic review in JPRAS Open of 24 studies (1,235 patients) looked at how long sagging correction lasts across techniques. Ask your surgeon what material they would use, how many they’ve done and how long they’ve followed their patients.

In my experience, the strongest predictor of a lasting lift is good skin quality and a stable weight, not a particular device. I’d rather explain that honestly than promise a result that depends on a mesh.

Results on darker skin

Breast lift on dark skin follows the same techniques, but raised scars and colour changes are more likely, so scar planning matters.

People with darker skin have a higher tendency to form raised (hypertrophic) scars or keloids and may see scars darken before they fade. Tell your surgeon if you’ve had raised scars before. Shorter-scar techniques, early scar care and sun protection can help. ASPS notes that scars are permanent but usually fade. Our breast lift scars guide covers healing stages and treatment options.

Frequently Asked Questions

FAQ

Answers on lift-only versus implant results, vacuum therapy photos, how long a lift lasts and when to judge the result.

What does a breast lift before and after with no implants look like?

Breasts sit higher with the nipple centred, look firmer and rounder, and may be slightly smaller. The upper breast gains a little shape but not the fullness an implant gives. Scars are visible early and fade over months.

How are results with implants different?

Adding an implant fills the upper breast and increases size, which a lift alone can’t do. Breast lift before and after photos with implants show more volume and projection. See our guide to a breast lift with augmentation.

Do vacuum breast lift before and after photos show a real lift?

Any change from suction therapy is temporary swelling, not a lasting lift. We found no good-quality evidence that vacuum devices raise sagging breasts long term. See our guide to a breast lift without surgery.

How long do lift-only results last?

Results are long-lasting, but breasts keep aging. ASPS says stable weight and a healthy lifestyle help, while pregnancy or large weight loss can reduce the effect. Some people choose a small revision years later.
